Permanently Disabled Absentee Voting Program

Voters who have a permanent disability or physical incapacity and will be unable to be present at the polls at any future election, may enroll in the Permanently Disabled Absentee Voting Program and automatically receive a ballot for the next 5 years.
Once enrolled in the program, the County Clerk’s Office automatically sends an Application for Absentee Ballot and ballot to the voter for every election in the five-year period following enrollment in the program.
Step 1
Please complete the Application for Absentee Voter’s 5 Year Identification Card/Affidavit of Attending Physician and sign it in the presence of a Notary Public. Also, your physician must complete the “Affidavit of Attending Physician” section in the presence of a Notary Public.

Mail completed application and affidavit to:
